Collaborative Approach to Recovery and Empowerment
(CARE+
SM)

The goal of the CAREModel is to provide individuals with a collaborative, comprehensive and whole-person treatment approach that incorporates psychosocial rehabilitation services, intensive case management services, individual and group treatment, and integrated medical/medication management services. The combination of skills training and case management services can help individuals with behavioral health conditions develop basic life skills, identify individual strengths, and improve their capacity to be successful in their life, work, relationships and community. CAREplaces special emphasis on Social Determinants in promoting health and health equity.

The CARE+ Model received an innovation grant from Stanford University Medicine and is nationally recognized for its successful outcomes, with an article published in the Journal of Health Affairs available here.

Integrated Care

People with behavioral health conditions and other psychiatric concerns may often need help in different aspects of their lives including work, living, social and learning environments. By utilizing a recovery model of behavioral health, our approach is centered on the individual's potential for recovery and focused on providing empowerment, self-determination, social inclusion, support, and various life skills. We are committed to providing services in as normalized an environment as possible using a strengths-based approach to care and treatment. CARE+ offers a whole person approach to identify root cause barriers affecting health and function and works collaboratively with the patient and medical provider to prioritize areas of focus. While full recovery is frequently the goal, we know that full recovery is seen as a process rather than an outcome.

Who We Serve

Individuals who might benefit from the program include, but are not limited to:

  • Individuals who, while functional, feel that they need additional support, assistance and recovery maintenance

  • Individuals who need help restoring back to their full level of functioning after treatment and/or placement in higher levels of care

  • Individuals who need ongoing assistance in multiple life domains (housing, employment/vocational skills, life skills, peer and social relationships, wellness, etc.)

  • Individuals who lack the supportive environment and resources they need to achieve their full level of functioning

  • Individuals who would benefit from community-based services

  • Individuals in need of access and linkage to various community resources

  • Individuals who are willing and able to participate in weekly group treatment

Outcomes

The CARE+ Model has been proven to positively impact patients' well-being. Some examples include:

  • Lowered hospitalizations and utilization of the ER

  • Reduced depression and anxiety as evidenced by the PHQ-9 screening tool 

  • Improved housing and employment security

  • Improved management of nutrition and issues related to diabetes

  • Improved access to health care providers and better management of chronic illnesses

  • Improved socialization skills and development of natural supports

  • Improvement in affect and mood and self reported increase in “happiness” and with “life"

  • Improvement in life management and independent living skills such as with household budgeting, activities of daily living, grocery shopping, etc.

  • Linkage with community resources such as access coordinators, disability benefits, and other healthcare and specialty providers

  • Improved compliance to daily medications

  • Improved adherence to healthcare visits
